Abstract Introduction Exploding head syndrome (EHS) is a hypnagogic parasomnia where the patient experiences loud noise or flash of light and often reports distress secondary to stimulus. EHS relatively uncommon, our knowledge, there has been only one published report comorbid narcolepsy, describing 38-year-old male. This focuses on pediatric case EHS, evaluating possible narcolepsy sleep apnea. Report Cases: Eight-year-old male was referred for excessive daytime sleepiness. He endorsed trouble staying awake in school, both morning after lunch time. His Epworth clinic 16, despite nightly duration ≥8 hours. also weakness strong emotions observed have difficulty grasping holding pencil while tickled. Patient his caregiver denied snoring, witnessed apnea, dream enactment, symptoms restless leg syndrome. However, did admit auditory hallucinations, occurring about twice per week during transition from wakefulness sleep. described hallucination as banging sound clinging bracelets, noting he searched source stimulus, but never able find cause. Patient’s mother hearing similar noises. No visual hallucinations paralysis noted. Polysomnography revealed 0 Central Sleep Apneas, Mixed 10 Obstructive 12 hypopneas. The mean these events seconds. patient’s Apnea-Hypopnea Index (AHI) 2.5 had REM AHI 8.3. supine 75% night. Conclusion illustrates unique patient. At this point, would suggest characterized by that involve sound, upon wake. exhibits some type - hypersomnia, subtle cataplexy. In lab PSG – showed overall 2.5, 8.3- oxygen nadir 90%.
